Hearing Development Questionnaire

Child Communication Development Questionnaire for Parents

The following questions reflect typical development of skills related to hearing and communication. If the answers for your child suggest that he/she is not developing these skills on schedule, mention this to your child's primary care clinician/medical home and ask about further testing for hearing.

2 Months of Age

  • Hearing
    • Have you had any worry about your child's hearing?
    • When your child is sleeping, does he move and begin to wake up to a loud sound?
  • Communication
    • Does your child smile at you when you are looking face to face?
    • Does he look at your face without you making gestures to him?

4 Months of Age

  • Hearing
    • Have you had any worry about your child's hearing?
    • When your child is sleeping, does she move and begin to wake up to a loud sound?
    • Does he try to turn his head toward an interesting sound or when his name is called?
  • Communication
    • Does she laugh and giggle without being tickled or touched?
    • Does he coo to himself and make noises when he is alone?

6 Months of Age

  • Hearing
    • Have you had any worry about your child's hearing?
    • When your child is sleeping, does she move and begin to wake up to a loud sound?
    • Does he try to turn his head toward an interesting sound or when his name is called?
  • Communication
    • Does she scream or squeal for fun or attention?
    • Does he use consonant sounds when babbling?

8 Months of Age

  • Hearing
    • Have you had any worry about your child's hearing?
    • When your child is sleeping, does she move and begin to wake up to a loud sound?
    • Does he try to turn his head toward an interesting sound or when his name is called?
    • Does she enjoy ringing a bell, playing with a noisy toy, or shaking a rattle?
  • Communication
    • Does he make a number of different sounds and change their pitch?
    • Does she clap her hands in imitation and make noises at the same time?

10 Months of Age

  • Hearing
    • Have you had any worry about your child's hearing?
    • When your child is sleeping, does she move and begin to wake up to a loud sound?
    • Does he turn his head toward an interesting sound or when his name is called?
    • Does he try to imitate you if you make his own sounds?
  • Communication
    • Does she play peek-a-boo with you?
    • Does he understand the meaning of any words (names, actions, etc.)?
    • Does she use repetitive sounds in babbling (mama, dada, baba)?
