Eligibility: Ages 0 - 18, children with or at risk of developmental delays. Medicare, sliding scale based on income, most insurance with the exception of SelectHealth, and Humana. Partially contracted with Educator's Mutual. Tri-Care requires a referral.
Services Provided: The UDAC is a center of excellence for the evaluation of children with or at risk for developmental delays, providing development assessments and management recommendations, partnering with primary care providers. Services include: Developmental Pediatrics, Psychology, Genetics, Ophthalmology, Neurology, Audiology, Speech and Language Pathology, Occupational and Physical Therapy, Care Coordination, Family Support Services.

Four programs of the UDAC are: Child Development Program, Neonatal Follow-up Program, Heart Center Neurodevelopmental Program, Orthopedic Follow-up Program.

Most programs offer clinics in Salt Lake and in 8 communities throughout Utah, termed "itinerant clinics," and/or "traveling clinics." Evaluation teams travel to sites in Blanding, Montezuma Creek, Moab, Logan (Neurology only), Ogden, Price, Richfield, St. George, and Vernal.
